PLI Revival Medical Form and Process
As you are aware recently Postal Life Insurance has revised the PLI revival form and PLI revival medical form. Here we will be providing the PLI revival medical form for your convenience.
Now the certificate from an immediate superior in the PLI revival form has been dispensed with by the PLI and you just need to download the PLI revival form and PLI revival medical form and get completed for submission to PLI-CPC.
PLI Revival Medical Form
Simply fill out the PLI/RPLI revival form and get the medical from the Doctors, preferably from the Doctors from Government Hospitals/dispensaries or Doctors empanelled by the PLI.
This PLI l medical form is available back side of the revival application form. In addition to this, you have to submit witnesses from two respectable persons not related to you. The format which is already available in the backside of the PLI revival form is as under for information:-

Process of Revival of PLI/RPLI Policy
For the revival of the PLI and RPLI process a detailed guide is already available, before submitting this form to any CPC i,e Head Post office please check there for revival guidelines and also calculate the revival amount of your PLI Policy.
After completion of this application form, submit this form to the nearest CPC/Head Post office. After 7-10 days you will receive a revival confirmation notice from PLI to deposit the revival amount. Submit this revival notice to the nearest Post office and deposit the amount.
Your policy will be revived. In case you have created the online PLI customer portal id, you can check the revival amount online or may visit the nearest post office to get the revival quote. Ensure to submit the application well in advance so that the revival application Form can be approved by PLI within a month.
In the future to avoid the lapsation of your policy always deposit the premium in advance or through online mode or Standing instructions from IPPB mobile app.
